En caso de que no exista plano\n\ncatastrado, el requisito debe ser\n  sustituido por un plano de agrimensura, que cumpla con lo estipulado\n\nen el artículo 2 inc. q) del Reglamento a\n  la Ley de Catastro Nacional.\n\nEl AyA contará\n  con un plazo de 20 días hábiles para resolver la solicitud.\n\n \n\nINSTRUCCIONES\n  PARA COMPLETAR EL FORMULARIO\n\nApartado I. DATOS DEL SOLICITANTE\n  (PROPIETARIO O REPRESENTANTE LEGAL)\n\nCasillas Nº 1 a la Nº 5: Completar la\n  información de la persona física que realiza la solicitud de\n\nPermiso de Descarga (propietario o\n  representante legal).\n\nCasilla Nº 6: Indicar la potestad que\n  tiene la persona indicada en la casilla Nº1 dentro de la asociación, sociedad\n  o fundación.\n\nApartado II. DATOS GENERALES DE LA\n  ACTIVIDAD\n\nCasillas Nº 7 a la Nº 13: Indicar el\n  nombre, razón social, cédula jurídica, correo electrónico, teléfono, fax y Número\n  de Identificación de Servicio (NIS) del Ente Generador que se responsabiliza\n  del (los) vertido(s).\n\nCasilla Nº 14 a la Nº 19: Se refiere a la\n  ubicación geográfica de las instalaciones en donde se originan las aguas\n  residuales, la dirección deberá contar con un punto de referencia fácilmente\n  identificable, como por ejemplo una escuela, iglesia, centro de salud,\n  estación de policía, edificios municipales y otros, además de anotar el\n  número de calles y avenidas respectivas.\n\nCasilla Nº 20: Indicar brevemente la\n  actividad principal del Ente Generador.\n\nCasilla Nº 21: Se refiere a la\n  Clasificación Industrial Internacional Uniforme (revisión tres), en la cual\n  se asigna un código que permite la clasificación de las diferentes\n  actividades económicas.\n\nCasilla Nº 22:\n  Indicar la jornada de producción diaria, semanal y mensual del Ente\n  Generador.\n\nCasilla Nº 23: Se refiere al número total\n  de empleados que permanecen en las instalaciones, desglosando los que\n  trabajan en producción, administración u otros.\n\nCasilla Nº 24: Indicar la procedencia de\n  las fuentes de abastecimiento de agua, tanto para consumo humano como para\n  procesos, incluyendo el caudal de agua requerida en cada caso y el caudal\n  total promedio del último semestre expresado en metros cúbicos mensuales (m3/\n  mes). En caso de que el Ente Generador posea concesión de aprovechamiento de\n  aguas, se debe aclarar que el caudal que se debe indicar no corresponde al\n  caudal concesionado sino al caudal real utilizado por el Ente Generador. En\n  caso de que la fuente de abastecimiento corresponda a un acueducto, el caudal\n  reportado se estimará a partir de volumen facturado.\n\nCasilla Nº 25: Indicar el destino de las\n  aguas residuales ordinarias, es decir, de las aguas generadas por las\n  actividades de los empleados (servicios sanitarios, comedor, lavatorios,\n  etc.).\n\nApartado III. DATOS GENERALES DEL PROCESO\n  PRODUCTIVO O ACTIVIDAD COMERCIAL (en caso de entes generadores aún no\n  instalados o establecidos, se deberá ingresar la información a partir de\n  datos proyectados o estimados)\n\nCasilla Nº 26: Indicar las etapas y\n  principales materias primas utilizadas en el proceso productivo, así como el\n  nombre del o los productos terminados.\n\nCasilla Nº 27: Indicar la cantidad, las\n  unidades y el nombre del producto terminado, se puede expresar en términos de\n  la materia prima procesada.\n\nCasilla Nº 28: Indicar los meses en los\n  cuales se mantiene la producción en la planta de proceso o actividad comercial.\n\nCasilla Nº 29: Indicar el punto de\n  descarga de las aguas residuales especiales generadas en los procesos de\n  producción.\n\nCasilla Nº 30: Se refiere al caudal\n  promedio vertido durante el último semestre, expresado en metros cúbicos por\n  mes (m3/mes).\n\nCasilla Nº 31: Indicar el tiempo promedio\n  de vertido al día expresado en horas.\n\nCasilla Nº 32: Indicar el promedio de días\n  al mes en que hay vertido.\n\nCasilla Nº 33: Indicar los meses del año\n  en los cuales existe vertido.\n\nCasilla Nº 34: Breve descripción del\n  sistema de pretratamiento de aguas residuales.\n\nCasilla Nº 35: Anotar los valores reales o\n  estimados de las aguas residuales producidas para cada uno de los parámetros\n  de calidad del agua residual solicitados por el Decreto Nº 33601-MINAE-S.\n\nCasilla N° 36: Se debe adjuntar un\n  diagrama de flujo en el que se indiquen las principales operaciones unitarias\n  ordinarias y de proceso productivo en las que se utiliza el agua. Se podrá\n  realizar a mano alzada o en computadora, en ambos casos deberá ser lo\n  suficientemente claro, lógico y legible de tal manera que permita su fácil\n  comprensión.\n\n \n\n \n\n \n\n \n\n \n\n \n\nNota: en caso\nde existir más de una descarga, se deberá presentar la información anterior\npara cada una de las mismas. En caso de Entes Generadores en operación y que no\ncuenten con Permiso de Funcionamiento, deberá aportar copia del análisis de la\ncalidad del agua residual vertida, emitido por un laboratorio habilitado (el\nanálisis no puede tener más de tres meses de realizado).\n\n36. If no cadastral map\n\nexists, the requirement must be substituted\n  with a survey map that complies with the provisions\n\nof article 2, subsection q) of the\n  Regulation to the National Cadastre Law.\n\nAyA shall have a period of 20 business days to resolve the request.\n\n \n\nINSTRUCTIONS FOR COMPLETING THE FORM\n\nSection I. APPLICANT DATA (OWNER OR LEGAL REPRESENTATIVE)\n\nBoxes No. 1 through No. 5: Complete the\n  information for the individual submitting the request for\n\na Discharge Permit (owner or legal\n  representative).\n\nBox No. 6: Indicate the authority held by\n  the person identified in Box No. 1 within the association, company,\n  or foundation.\n\nSection II. GENERAL DATA OF THE ACTIVITY\n\nBoxes No. 7 through No. 13: Indicate the\n  name, corporate name, legal identification number, email address, telephone\n  number, fax number, and Service Identification Number (NIS) of the Generating\n  Entity that is responsible for the discharge(s).\n\nBox No. 14 through No. 19: Refers to the\n  geographic location of the facilities where the wastewater originates; the\n  address must include an easily identifiable reference point, such as a\n  school, church, health center, police station, municipal buildings, and\n  others, in addition to noting the respective number of streets and avenues.\n\nBox No. 20: Briefly indicate the main\n  activity of the Generating Entity.\n\nBox No. 21: Refers to the International\n  Standard Industrial Classification (Revision three), in which\n  a code is assigned allowing the classification of the different\n  economic activities.\n\nBox No. 22:\n  Indicate the daily, weekly, and monthly production schedule of the Generating\n  Entity.\n\nBox No. 23: Refers to the total number of\n  employees present at the facilities, breaking them down into those\n  working in production, administration, or other areas.\n\nBox No. 24: Indicate the source of the\n  water supply sources, both for human consumption and for\n  processes, including the water flow rate required in each case and the total\n  average flow rate for the last six months expressed in cubic meters per month (m3/\n  month). If the Generating Entity holds a water use concession,\n  it must be clarified that the flow rate to be indicated does not correspond to\n  the concessioned flow rate but rather to the actual flow rate used by the Generating Entity. In\n  the event that the supply source corresponds to an aqueduct, the\n  reported flow rate will be estimated based on the billed volume.\n\nBox No. 25: Indicate the destination of\n  ordinary wastewater, that is, the water generated by the\n  activities of the employees (sanitary services, dining hall, washrooms,\n  etc.).\n\nSection III. GENERAL DATA OF THE PRODUCTION\n  PROCESS OR COMMERCIAL ACTIVITY (in the case of generating entities not yet\n  installed or established, the information must be entered based on\n  projected or estimated data)\n\nBox No. 26: Indicate the stages and\n  main raw materials used in the production process, as well as the\n  name of the finished product(s).\n\nBox No. 27: Indicate the quantity,\n  units, and name of the finished product; this may be expressed in terms of\n  the processed raw material.\n\nBox No. 28: Indicate the months in\n  which production is maintained at the processing plant or commercial activity.\n\nBox No. 29: Indicate the discharge point\n  of the special wastewater generated in the production\n  processes.\n\nBox No. 30: Refers to the\n  average flow rate discharged during the last six months, expressed in cubic meters per\n  month (m3/month).\n\nBox No. 31: Indicate the average\n  discharge time per day expressed in hours.\n\nBox No. 32: Indicate the average number of days\n  per month on which discharge occurs.\n\nBox No. 33: Indicate the months of the year\n  in which discharge occurs.\n\nBox No. 34: Brief description of the\n  wastewater pretreatment system.\n\nBox No. 35: Note the actual or estimated\n  values of the wastewater produced for each of the wastewater\n  quality parameters requested by Decree No. 33601-MINAE-S.\n\nBox No. 36: A\n  flow chart must be attached indicating the main ordinary unit\n  operations and those of the production process in which water is used. It may be\n  done freehand or by computer; in both cases, it must be sufficiently\n  clear, logical, and legible so as to allow for easy\n  comprehension.\n\n \n\n \n\n \n\n \n\n \n\n \n\nNote: If\nthere is more than one discharge, the above information must be submitted\nfor each one. In the case of Generating Entities that are in operation and do\nnot have an Operating Permit, they must provide a copy of the analysis of the\ndischarged wastewater quality, issued by an accredited laboratory (the\nanalysis cannot be more than three months old).\n\n36. Attach a flow chart in which the main ordinary unit operations\nand those of the production process in which water is used are indicated.\n\nThe applicant\ndeclares that the data provided are faithful and true and has given them\nunder oath, likewise, commits to comply with all provisions\nof Decree 33601-MINAE-S (Regulation of Discharge and Reuse), and therefore,\nFORMALLY REQUESTS the Wastewater Discharge Permit to the Sanitary\nSewer System administered by AyA.\n\n \n\n \n\nAgreement No.\n2019-461, adopted by the Board of Directors of the Instituto Costarricense de Acueductos\ny Alcantarillados, in subsection 5.1, of article 5, of Ordinary Session No.\n2019-72, held on November 26, 2019."
